Follicular Unit Extraction, FUE, is the extraction of hair grafts as individual hair follicles in a two-step or three-step using micro drill gadget for making small micro punches of size 0.6–0.8 mm. In this technique, the hair follicles have extracted a region from the donor region and are implanted to the recipient sites singly. It’s a blind technique as no aid is used to enhance magnification while extracting the graft from the donor region. So, the transection rate or the damage rate of roots is high because of roots of hair of scalp which may vary from straight to inclined.

The cost of FUE in India is usually higher than FUT, the strip method as the time taken for extraction is more (For extracting 2500 grafts it takes almost 10-12 hours). Each graft charge is approximately 40 INR to 120 INR depending on the donor region from where the hair grafts are being extracted. The overall cost depends on the age, the degree of baldness, the recipient site (Vertex region is the most complex region in hair transplant where a whorl-like a pattern is found, the temporal region is the other most challenging region) and the number of needed grafts.

Follicular unit extraction (FUE) is a technique of harvesting hair grafts whereby punches of several types are used to remove follicular units from the donor region one at a time. The main advantages of this technique to patients are predominantly the lack of a linear scar. In general, the FUE hair transplant technique allows patients to completely shave during the procedure. Local anesthesia is used to numb the bald region. Local anesthetic is given to control discomfort or pain during surgery. Tumescent in addition to normal saline (NS) is given to allow the follicular units to implant in an easy manner. There are two basic punch types used to perform FUE, sharp and dull tips, and within each category, there are manual and powered versions. The FUE technique performing sharp dissection typically involves limited depth punch insertion to decrease the risk of follicle transection. The FUE technique performing blunt punch dissection allows for an intense level of dissection, thereby decreasing the force required for graft removal.

There are several signs for using FUE in restorative and repair procedures. In general, any patient who is a candidate for hair restoration by the strip method is a candidate for FUE, as the cosmetic results in the recipient region are the same as in strip surgery. Additionally, there are probable candidates for FUE restoration who are not candidates for strip harvest by virtue of low donor hair density and how short they would like to wear their hair. A patient having very fine donor hair and low donor density may be a poor candidate for a strip harvest, yet may achieve a good result with FUE. The probable candidates and most patients seeking FUE are those who desire the option of wearing their hair short. Minimal postoperative pain and discomfort is certainly the desired outcome, but it is not usually the primary motivating factor for having FUE.

• The bald region needs to be completely shaved.
• Only 2500 hair grafts can be extracted due to Safe Area or Donor Area complexity which may arise on over-extraction through FUE for future sessions.
• High transection rate due to roots direction. It’s a blind technique in which no aid is used during extraction.
